
Important Changes for Patients with Humana Coverage *Action Required*
We want to keep you informed about our ongoing contract negotiations with Humana. We understand you may have received a letter from Humana, but please know that we are actively working together to prevent any disruption to your care.
If an agreement is not reached, our contract with Humana will officially end on May 1, 2025, meaning Commonwealth Pain & Spine will no longer be in-network with Humana Medicare Advantage or Humana Healthy Horizons Medicaid. However, this does not mean you have to lose your doctor, you have options to continue your care with us.
Please note: This does NOT impact our locations in North and South Carolina.
You do not have to change your doctor, and you can continue to see your CP&S physician and care team; however, you must take one of the following actions to continue your care at CP&S.
What Are Your Options?
- Stay with Commonwealth Pain & Spine using out-of-network benefits. Contact us to see if your plan allows this option if you’re unsure.
- Switch to a different Medicare plan. You may be eligible to switch back to Traditional Medicare or choose a new Medicare Advantage plan that includes Commonwealth Pain & Spine in-network. To request a Special Enrollment Period, call Medicare at 800-633-4227 (TTY: 877-486-2048).
- Change Medicaid plans. If you are a Humana Healthy Horizons Medicaid member, you can explore other plan options by calling 1-855-446-1245 or 1-800-635-2570.
- Continuity of Care (COC) Forms - If you received a letter from Humana, you should have also received a Continuity of Care (COC) request form. Please complete and return it as soon as possible to Humana. Completing this form may allow you to continue receiving in-network care for up to 90 days. If you have not received a letter or COC form, please contact Commonwealth Pain & Spine, and we will provide one for you.
